Our medical centre provides a range of clinics and services to all our patients.

The links below take you through some of the services available. If you would like more information please get in touch.

Test Results

Annual reviews - If your test results are within the expected range or there is no follow up action to take we do not routinely get in touch to let you know. If your test results show that you need more tests or treatment, we will contact you.

Urgent and follow up test results - the GP who requested the tests will agree a plan with you.

Once a doctor has reviewed your test results, you can view them:

  • in your NHS account (through the NHS website or NHS App)
  • by phoning us on 01969 622391 and we will tell you what the results are
Sick (fit) notes

If you’ve been off sick for more than 7 days

To request a sick note:

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

GP referrals

To request a GP referral, arrange an appointment.

NHS e-Referral

If your doctor has referred, you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system. Log in to manage your referral.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

Clinics

Child services

8-week checks take place with a GP and a nurse.

Routine immunisations take place with a practice nurses.

Family planning clinic

We have a family planning clinic every Monday.

The clinic covers coil and implant insertion and removals.

Travel advice/immunisations
  • The practice provides appointments for travel advice/immunisations
  • You should complete and submit a travel questionnaire and we will call you to make an appointment with a practice nurse
  • Please note it is recommended that immunisations should be given at least 8 weeks before travelling
  • The practice cannot provide this service last minute
Mental health

The practice can offer help and treatment for mental health problems. Please make an appointment with our Primary care mental health worker.

Referral onto specialist mental health services is available if appropriate.

These include:

  • Counsellors
  • Community mental health teams
  • Clinical psychology
  • Consultant psychiatrist
Chronic disease clinics
  • Many people have ongoing chronic health problems
  • These conditions need regular review (at least once a year), and the practice will send you an appointment for this
  • We also encourage self-management and appointments in the clinic if you feel you have any problems
Long term conditions

Many people have ongoing long term health problems.

These include:

  • Diabetes
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Asthma
  • Heart and kidney disease

These conditions need regular reviews at least once a year. If you have a long term condition, you will receive regular invites to the practice for reviews.

Over 40s health check
  • Helping you prevent heart disease, stroke, diabetes and kidney disease
  • Appointments are with our practice nurse and health care assistant

No-Scalpel Vasectomy

We are a contracted provider of NHS no-scalpel vasectomy services in Hambleton, Richmondshire and Whitby and Harrogate and District. We are also a contracted provider for the Durham Dales and for the armed services in the North.

The service is carried out by doctors who are members of the British Association of No-Scalpel Vasectomists (BANSV). The majority of clinics are held at Leyburn.

If you are interested in having a no-scalpel vasectomy and you are a Leyburn Medical Practice patient, then please make an appointment to see a GP who will be happy to discuss the procedure with you.

If you are not a Leyburn Medical Practice patient, but you live in North Yorkshire or the Durham Dales, or you serve in the armed forces, then please ask your GP to either email a referral to Leyburnvasectomy.service@nhs.net or book via the e-Referral Service.

We are a veteran friendly accredited practice.

When you join our practice identify yourself as a veteran, being able to identify and support veteran patients is crucial to ensure best outcomes, as they may face different health challenges to the general population.

Non-NHS Work

The government’s contract with GPs covers medical services to NHS patients. In recent years, more and more organisations have been involving doctors in a whole range of non-medical work.

Examples of non-NHS services for which GPs can charge are:

  • Certain travel vaccinations
  • Private medical insurance reports
  • Holiday cancellation claim forms
  • Referral for private care forms
  • Letters requested by, or on behalf of, the patient

Examples of non-NHS services for which GPs can charge other institutions are:

  • Medical reports for an insurance company
  • Examinations for local authority employees
